Silicone Breast Implants On November 17,2006 the FDA approved the use silicone breast implants. Any woman over the age of 22 or any woman having breast reconstruction will have the option of silicone gel breast implants. Juvéderm The long awaited new filler product, Juvéderm, has received FDA approval to be used in The United States. Juvéderm is a colorless hyaluronic acid gel that is injected into facial tissues to smooth wrinkles and folds. Hyaluronic acid is a naturally occurring sugar found in the human body. The role of hyaluronic acid in the skin is to deliver nutrients and to hydrate the skin by holding in water. Although Juvéderm is similar to Restylane, there appears to be several advantages to using Juvéderm over other fillers. The fact that the hyaluronic acid is more highly cross-linked makes Juvéderm smoother to inject. This ultimately leads to more precise correction of wrinkles and folds. Juvéderm also appears to last longer than Restylane and other hyaluronic acid products. Dr. Mike is currently working with Allergan, the company that makes both Botox Cosmetic and Juvéderm, to further evaluate longevity and patient satisfaction with Juvéderm. Studies already show it will last 6 months but new finding suggest Juvéderm may last for 9-12 months after injection. How healthy is your facial skin? Dr, Mike spoke on midface lifting procedures and plastic surgery office management issues. The meeting was attended by approximately 900 plastic surgeons, dermatologists, and facial plastic surgeons from around the world.
Fat is Back! Fat Transfer that is... Much of the discussion at the 9th International Symposium centered around the need for volume replacement for facial rejuvenation. Youthful faces are distinctly round and full around the eyes, cheeks, and along the jawline. As we age we lose fat in our face. This process may be accelerated in women because of hormone fluctuations. Typically this fat loss occurs around the eyes, under the cheeks, and around the mouth. Fat loss contributes to the long tired look we associate with aging. Surgical lifting procedures such as facelifts, eyelid lifts, and forehead procedures address one component of aging which is loose skin. However, oftentimes these procedures fail to address the volume loss. Fat transfer surgery can be a useful adjunct to restore some of the youthful fullness to the face. Fat transfer has been around for years with varying amounts of success. Some of the transferred fat invariably will not survive. The trick is how to improve its survival. Newer harvesting and transfer cannulas have now been developed which make the procedure less invasive while at the same time improving the result. Fillers: Restylane and Radiesse have become increasingly popular fillers in the Unites States. These synthetic compounds are very similar to other chemicals we already have in our bodies so adverse reactions from either product are extremely unlikely. Restylane is best used for superficial lines around the mouth and for lip augmentation, whereas Radiesse works best in deeper furrows and creases. New fillers such as Perlane, Artfill (Artecoll), and Bio Alcomid are being used outside of the United States with good success and longevity. Studies are on going with our FDA for each of these products as well as other promising fillers. Hopefully we will soon see safer, longer lasting filler products become available to the US market. Light Therapy: Photo dynamic therapy (PDT) is being combined with intense pulsed light (IPL) to significantly improve skin texture and color. With Photodynamic therapy a chemical is applied to the skin surface. This chemical is only absorbed by damages skin cells. The chemical becomes activated by light exposure. Damages skin cells revert back to normal and new collagen is developed. The result is smoother, fresher, more youthful appearing skin. Brown spots and sun damage disappear.
Thread Lifts:
The jury is still out on thread lift surgery. Experiences surgeons from around the world who have tries these procedures report results which are very subtle and fail to last more than two years. The hazard of course is having a foreign body suture material under the skin which will always be a possible source for rejection. In fact, the quick lift is a modification of a procedure that was performed nearly a decade ago and abandoned, because the results were not natural appearing and did not last. Drs. Sullivan do not perform, nor do they recommend the quick lift procedure as a viable option for face-lifting. THREAD LIFT:
The thread lift procedure was invented in 1999 by Russian cosmetic surgeon, Dr. Marlen Sulamanidze. The skin in this procedure is pulled up by using aptos threads. The threads are similar to high-test fishing lines with barbs. Once inserted under the skin, the barbs can be used to elevate the skin and soft tissue. Dr. Sullivan has seen this procedure performed on several occasions in the laboratory and does not advocate this procedure for a number of reasons. The skin and soft tissue are pulled, resulting in unnatural gathering and puckering. Since this is a new procedure, there is no long term data available about the aptos threads. Recent evidence suggests the threads can become infected and extrude through the skin. In fact, we are suspect of the degree and length of improvement, because the barbs will tear through the skin and tissues. WRINKLE FILLERS: Every year the media hypes the latest advances in plastic surgery products and services as the new fountain of youth. Last year it was BOTOX® Cosmetic. This year the hype is all about "wrinkle fillers", paste and gel implants, which can be injected beneath the skin to erase folds and wrinkles. Think of fillers like caulking material. They plump up the skin. Collagen has been used for many years. Recently a number of newer fillers with distinct advantages over collagen have received FDA approval and are now available. All of these newer fillers have been used in Canada and Europe for years with good success. Upon consultation, your plastic surgeon will be able to recommend the filler that is best suited for your areas of concern. None of the newer filling agents contain local anesthetic. In most cases either a topical cream is applied to the area or a nerve block is performed prior to injecting any of the newer fillers. The most common side effects from any of the newer fillers are temporary and mild in intensity including swelling, redness, tenderness, and bruising. To minimize the risk of bruising, patients should avoid taking aspirin products, and other anti-inflammatory agents, and vitamin C and E supplements one week proir to the injection. COLLAGENTM : Collagen is the original filler. It comes form an animal source therefore posing the risk of allergic reaction. It is necessary to have a skin test to check for sensitivity. This may not be the most convenient since you must wait weeks for the results of the skin test before having the actual injection. This product usually lasts 2-3 months. RESTYLANE TM: Restylane is a safe and natural cosmetic dermal filler that restores fullness to the skin to eliminate facial wrinkles and folds. It is made up of synthetic hyaluronic acid, a natural substance that already exsists in the human body. For this reason, you will not be allergic to Restylane and a skin test is not necessary. Restylane results usually last for 4-6 months. RADIESSETM : Like Collagen and Restylane, Radiesse is also a filler substance but it differs in it's consistency and duration of results. Radiesse is considerably thicker and is injected beneath the skin surface to elevate the tissues of deeper creases. The most common location for this injection are the nasolabial folds or smile lines around the mouth. It can also be placed into the skin or cheeks to enhance their appearance. Made from calcium hydroxyapatite (a natural mineral in the body), you cannot be allergic to Radiesse. The results should persist for 2-3 years producing a natural, rejuvinated appearance. HYLAFORM : Hylaform is very similar to Restylane but made by a different company. It too is made up of synthetic hylauronic acid but also contains a small amount of avian protein. Its use is contraindicated for patients with a known history of hyper-sensitivity to avian proteins. On~Q Pain Pump: The ON~Q pump device is the latest innovation in pain relief management after breast augmentation surgery. A small catheter inserted at the time of surgery provides effective pain relief with "numbing medication" which significantly reduces the need for postoperative pain medication. The ON~Q pump is completely portable, safe and disposable. It gives our patients continuous relief for up to three full days, which means returning to normal activities sooner. Micro-dermabrasion removes the outer most layers of skin leaving a smoother, fresher and more youthful appearance. Our new crystal-free machine gently removes and vacuums away unwanted dead skin cells without the annoying grit of sand and crystals.
